Dr Joel Rhee is committed to providing proactive and preventative care to his patients. He therefore recommends the following for all his patients.
- Annual Comprehensive Health Assessments.
Comprehensive history and physical examination to identify new problems and unmet care needs. An experienced registered nurse may assist Dr Rhee in conducting these assessments. - Annual medication reviews.
These are conducted in collaboration with a pharmacist who reviews the patient’s medications. Dr Rhee will then discuss any recommended changes with the patient and/or the person responsible. - Contribution to a multidisciplinary care plan (once or twice a year).
Dr Rhee will review and contribute to the care plan developed by the aged care home. - Multidisciplinary case conferencing (on a need-to-basis).
This involves Dr Rhee speaking with the care providers and other health professionals involved in the care of the patient to ensure that the patient’s care needs are being met. This can be done in person or via telephone/videoconferencing.
The above are covered by Medicare. Private fees apply for people without a Medicare card.
